Total hip replacement (THR) results with the computer navigated cup

Introduction: The cup orientation has a significant influence on the outcome of the THR. In the last years several CAS-systems have been developed to improve the accuracy of the placement of the cup prosthesis into the acetabular part during surgery. The purpose of this paper is to represent our first experiences with the SurgiGATE-system for computer navigated cup placement. Materials and Methods: Between July 1999 and September 2002, we performed the cup placement with several CAS-systems in 118 patients, including 31 patients we used the SurgiGATE-system (Medivision, Oberdorf, Switzerland). The 31 THR, based on the SurgiGATE-system, were performed by 3 surgeons using 2 different types of uncemented cup implants. There were 20 female and 11 male patients. The average age of the SurgiGATE navigated group was 63 years. The SurgiGATE-system provides the surgeon with the possibility to plan and implant the cup with correct inclination, antetorsion, depth and size. The following study describes problems during planning and performing the cup placement. A functional evaluation was done after the Harris-hip-score. Results: In 4 cases the navigation was converted into a free-hand cup placement because of some different reasons. We registered various problems during the scenario of matching and navigation in 5 patients. 20 cup placements occurred without any difficulties. In these cases the matching scenario and the followed navigation was exactly. The surgery time was in average 115 minutes. The assessment after the Harris-hip score was significantly better at the time of follow up compared with the data preoperatively.
