Acute renal failure (ARF) occurs when there is an abrupt and prolonged decline in glomerular filtration rate resulting in an accumulation of nitrogenous wastes. Dogs and cats with ARF may have reversible nephron damage; aggressive treatment may be sufficient to prevent permanent nephron loss and irreversible renal failure. The differentiation of ARF and chronic renal failure (CRF) can be complicated and the clinical presentation of acute-on-chronic disease often resembles ARF. Despite the poorer short-term prognosis, dogs and cats with ARF that respond to aggressive medical management may do very well. In one study of 99 dogs with ARF, although 56% of dogs died or were euthanatized during initial hospitalization, 19% of dogs had return to normal serum creatinine concentration, and 24% developed CRF.1 Recognizing and correcting causes of acute renal damage as well as prompt and appropriate management of the ensuing failure is important to prevent or minimize irreversible damage in patients with ARF.
