Organizing a network for integrating shared care management of obesity and type 2 diabetes

Obesity is highly prevalent in many societies and is particularly associated with type 2 diabetes. Given the importance of the stakes, in terms of health as well as of costs, a joint initiative of the local health insurance social security system, the hospital based Nutrition Service and hospital based Diabetology Unit originated in the "Obesity/Diabetes network" project in the Northern part of France. The major purpose of the project is the prevention of obesity and treatment of patients.. The success of this project is conditioned by the efficiency of the information system which sustains it. This paper is limited to treatment information. We propose a primary project specification by representing the system using the Unified Modeling Language (UML). Due to the high level of distribution and flexibility required, we adopt a component-based approach to design the network system. The superposition of three levels of components which appear in the generic model conditions the designing of the architecture capable of supporting the project.
