Unrelated Donor Stem Cell Transplantation: A Registry Perspective

Worldwide, more than 5 million volunteers have been HLA-typed as potential hematopoietic stem cell (HSC) donors for patients who otherwise would not have the option of transplant therapy. The organization and responsibilities of the more than 43 registries vary significantly, generally dependent upon their financial support. Until recently HSC donor recruitment has embraced a "come one, call all" philosophy. However, (1) the overall lower likelihood of a match for patients for minority populations, (2) the increasing redundancy of donors with common HLA phenotypes, (3) the better understanding of HLA population genetics, and (4) the need to develop cost-effective recruitment all argue strongly for developing scientifically based, internationally coordinated donor recruitment.
