FRACAS and Lessons Learned in Failure Tracking

摘要

In aviation, the goal of the System Safety Analyses and Reliability Analyses is to decrease the accidents. Bases of System Safety Analyses and Reliability Analyses are hazards and failures. However, these are not enough to decrease the accidents, the process about tracking the hazards and failures is necessary. FRACAS is a tool to help the organizations. Learning from failure is important to improving safety. Just like how joints in human body compliment the body, conducting these processes also play a role in providing both the quality obtained and the aimed safety level of the product.
