摘要

This paper presents a descriptive method to analyse the consequences on system safety due to human unreliability. System functions have to be identified and human implication to achieve some of them is defined in terms of procedures, i.e. lists of tasks to be performed related to work contexts in normal and abnormal system functioning. A failed task can be caused by three different behavioural factors: an acquisition related failure, a problem solving related failure and/or an action related failure. Consequence analysis consists of identifying consequence level of human unreliability. This method is then applied to the rail traffic.
