Background: Plantar warts, infected by human papillary virus, occur in the heel,metatarsal head or metatarsal. Presently, the methods of treating plantar warts includecryotherapy, local injection of pingyangmycin, etc. However, these treatments havemany problems such as severe pain, poor transdermal absorption and failure to reducerecurrence rate. In clinical practice, we found that for some refractory warts, surgicalcurettage combined with photodynamic therapy can be an ideal effect.Methods: We selected patients suffering from multiple (more than 3) plantar wartswith large area and unhealed ones. Operation steps: 1) Removing skin's keratin layerand warts. 2) Photosensitizer was given immediately after operation.3) Semiconductorlaser irradiation after light avoidance. 4) Curative effect. 5) Follow-up: the curedpatients were followed-up once a month for 6 months. Every time asked if anyadverse reactions occurred.Results: Among 93 patients, Improvement rate:100%,Recurernce rate:2.15%,Effective rate:97.85%. Some of the patients had a local burning sensation anddeveloped mild and moderate redness after irradiation. In some patients, the wound healing time was prolonged, and 3 patients recovered during 15 days after operation,most of them recovered for 5 to 7 days.Conclusion: Surgical cutting preconditioning combined with photodynamic therapyis a new method for treating warts. It is simple, practical and safe,and the recurrencerate of patients is greatly reduced and the curative effect is good.Especially for patients with multiple, large area refractory plantar warts, the way ofsurgical cutting preconditioning combined with photodynamic therapy can be givenpriority.
