Potential Severity Rate as a Leading Indicator: One Year Later

The Potential Severity Frequency Rate (PSFR) defined in this paper classifies all reported safety events (accidents and incidents) using defined levels of severity that are based on incident potential rather than actual outcome. These reported safety events include: lost time accidents (LTAs), non-lost time accidents (NLTAs), and potentially serious safety incidents (PSSIs) otherwise known as serious near misses. Our goal in introducing PSFR as a leading indicator is to reduce the number and frequency of injuries that occur in our operations. Currently, those incidents are measured by traditional lagging indicators of LTA and NLTA frequency rates (FRs). This paper is a follow-up to a 2012 case study and now includes an additional year of data. After 2.5 years, a link is emerging between the injury frequency rate (actual outcome) and potential severity of each incident. In order to continue and sustain this positive trend, the behavioral concept of Actively Caring Together will be introduced and paired with strong positive reinforcement, requiring buy-in at all levels, from senior leadership to front line employees and subcontractors. In this environment, all employees are encouraged to speak up and look out for the best interest of each other. This paper will present the following:1. An overview of the Potential Severity (PS) concept; 2. The introduction of PSFR as a leading indicator 3. The application of PSFR as a leading indicator across Air Liquide's Global E&C Solutions' operations;4.The results achieved after one year of implementation and 2.5 years of data trending 5.Introduction to the behavioral concept of Actively Caring Together.
