摘要

Japan participates in the International Space Station (ISS) Program with the Japanese Experiment Module (JEM). Alarge number of anomalies have occurred in JEM development. Many of them are found to be caused by not technicalissues but human errors. Moreover, management issues such as inadequate structuring, team member reduction withoutadequate hand over, lack of communication, etc are found behind those human errors. It is important for riskmanagement to identify risks behind signs and deal with them promptly. This report introduces the analysis method of“thread of connection” to identify fundamental causes behind those anomalies and measures to prevent recurrences.
