An Exercise in Mistake-Proofing and Failure Mode Effects Analysis

This paper discusses an exercise in mistake-proofing and failure mode and effects analysis (FMEA) titled "The Die Cast Factory." The exercise provides an opportunity to explore how product designers can anticipate and manage the process employed by users. FMEA is used to identify mistake-proofing opportunities. Process "failure modes" must be mistake-proofed to insure users do not injure themselves.
