An organization's safety culture is often the dominant force behind its safety performance and consequently, accidents. It has become increasingly apparent as systems become more complex that we cannot be limited to just the technical aspects of safety, risk and failures. We have to look at the sociotechnical hazards that appear in large-scale organizations and are implicit in their design. This notion emphasizes the complex interaction between a technology and an individual, group, or organization. In addition, recognition, and consequently the incorporation and management of cultural factors into systems represents another tool to contribute to the system's success.
