Analysis of infringements or potential inefringements of criticality rules

Data have been gathered, primarily from the UNited Kingdom and US, on occurrences of infringements or potential infringements of criticality rules over a limited period. This paper analyses these infringements, categorises them in terms of potential seriousness and attempts to identify if there may be a common cause or theme which may be relevant. The causes and effects of these infringements are then assessed and grouped into broad categiories of similar significance. The aim of this work is to identify any lessons that can be learned from past infringements which, in turn, culd results in improved operator control and allocation of limited specialist criticality assessment resources.
