An accident investigation technique used in a variety of industries (the Systematic Causal Analysis Technique) can be converted to use as a proactive hazard identification tool.The hazard identification is conducted using a What If question format (e.g."What if the line has not been properly isolated?").The review is given structure and thoroughness by examining an activity at each level of possible intervantion: (a) the loss itself,(b) the incident,(c) the immediate cause,(d) the basic causes,(e) lack of control.Each level in the hierarchy of the Loss Causation Model is provided with a list of prompts,analogous to HAZOP guide words,to assure complete coverage.Originally designed for power line workers,this approach is highly versatile and can be utilized for identifying hazards resulting from maintenance activities,machinery operations,field activities,mechanical shops,construction and similar activities.It can be effectively used both in a team setting and by individuals working in isolation.The method is systematic,relatively easy to understand,and yields consistent results.
