A system, device and method for removing an obstruction from an internal lumen, provided with a catheter, the distal end of the catheter being fluid-fillable, operably connected to at least one anterior focusing reflector. A litho cushion is attached. The catheter comprises a lumen and an electrode-to-housing is arranged at or near the distal end of the lumen so that it engages the lumen in a watertight manner. The electrode pair housing includes at least one electrode pair. At least one pair of electrodes is connected to the pulse generator via wiring. The electrode pair is configured to generate a shock wave by generating an electric arc between the electrodes of the electrode pair. The shock wave is directed distally outward from the catheter lumen, focused by at least one reflector, and directed distally away from the catheter lumen to the target obstruction.
