A writing tablet with fixed and/or removable pages and with hard covers, mounted on a base and supplied with a pen, has special pages, each formed into two separate leaves on which can be recorded messages, or marked with notes or reminders, so that replies can be entered on the second portions in answer to enquiries or notes on the first portions. The portions carrying the answers can be adhesive coated so that those portions can be removed, folded and sealed to keep the information concealed from other persons. The tablet is intended to be of appointments, treatment etc is entered, and where replies by doctors and other medical staff are entered. The tablet can incorporate further sections especially marked, coded or formed to convey information to the users.
