The invention relates to a printed form intended to facilitate scrutiny of a credit request. The granting of credit is subject to a certain number of conditions to each of which a number is attached; the request is favourably received if the sum of all of these numbers exceeds a predetermined threshold. The printed form comprises at least one set 1; 2 of three tables 11, 12, 13; 21, 22, 23 each with two entries. Each entry on the first two tables 11, 12; 21, 22 corresponds to the various possible states of a condition and the inscriptions carried in the boxes of each of these first two tables represent, in encrypted form, the sum of the numbers corresponding to the two states that this box represents. The third table 13; 23 comprises a number of columns and lines equal to the number of boxes respectively of the first and of the second table. Each box of the third table contains the number, in clear, representing the sum of the four states to which this box corresponds. IMAGE
