PURPOSE:To smoothly execute the operation of a check and to improve health consultation efficiency by providing a recording medium to record the personal identification information of a person to receive consultation and providing a part, to which the reception of consultation/no-reception of consultation is impressed for each check index. CONSTITUTION:A consultation receiving card reading and recording device receives a consultation receiving card by a carrying part 61 and carries the card to a printing part 62, which prints a character or a symbol on a printing surface, or a reading and storing part 63 to read data, which are already recorded, from a track for magnetic recording or to record new data. Then, the card can be discharged and respective elements receive control from a control part 64. Further, for a total health consultating system, a consultation receiving card reading and recording device 6 is respectively set in correspondence to a reception counter, respective checking rooms and checking equipments. Then, the device 6 is connected with a computer main body 3 and another terminal equipment.
