PURPOSE: To reduce the cost by laminating a natural fiber cloth on one of the surface layers made of the thermoplastic synthetic resin, and adhering the thermoplastic synthetic resin layer outside on the concrete placing side of a form. ;CONSTITUTION: In a surface material 3, a natural fiber cloth 1 is laminated on one of surface layers 2 made of the thermoplastic synthetic resin, and the cloth 1 is adhered to a concrete placing side of a form 5 using the adhesive. The surface material 3 is adhered on the concrete placing side of the form 5 with the surface layer 2 outside, and the form 5 which is excellent in the separability and does not damage the appearance of the concrete surface is manufactured at a low cost. The plain weave of the vegetable fiber is preferable for the kind of the natural fiber. The material of the thermoplastic synthetic resin of ≥100°C is preferable, and, for example, the olefin resin such as high density polyethylene which does not generate toxic gas when being burnt is used.
