PROBLEM TO BE SOLVED: To reduce the burden of production of an application form and also to reduce the burden of electronic jobs for check of the application forms and the generation of data at a university by distributing the application forms and the check programs via a storage medium and by accessing them via the electronic mails. SOLUTION: An application form/check program editing part 103 edits the contents of a storage means 101. An applicant sets a storage medium 108 storing his application form and input check program to a processor 110 and performs an input operation of the application form. Then this input operation is checked. A completed application form is stored in the medium 108 such as a floppy disk, etc., and sent to a university or sent to the university through an electronic mail, etc., via a public circuit network 109. An application check part 104 of a host computer 102 of the university adds an examinee's number, etc., to the application form data received via the medium 108 or an electronic mail control part 106. Then an application master production part 105 stores the contents of the application form in an application master 107.
