METHOD FOR FORMING SMOOTH-MUSCULAR SPHINCTER IN CASE OF PERITONEAL-ANAL RESECTION OF RECTUM

FIELD: surgery. SUBSTANCE: proposed method resides in mobilizing and moving downwards upper-lying regions of descending colon into anal canal so as to provide colon tissue excess. Next graft is tailored on peduncle formed by serous-muscular layer of descended intestine, graft is wrapped around distal part of descended intestine, thereby forming smooth-muscular sphincter, central portion of which is attached by suturing to edge of anal canal along resection line so as to leave excess of mucosa. Proposed method is indicated for use in cases of rectum diseases, in which it is necessary to resort to peritoneal-anal resection of rectum and internal sphincter, followed by moving upper-lying regions of descending colon down to anal canal. EFFECT: method was used clinically on 30 patients and gave good immediate and remote results; improved sphincter function.
