PROBLEM TO BE SOLVED: To reduce the size and weight of a refractive index medium and to obtain high diffraction efficiency even if a hologram photosensitive material is formed to a smaller thickness by making reproducing light incident on the main surface of the refractive index medium from the end face of the refractive index medium at a prescribed angle with the normal of the hologram film formed at a prescribed thickness on the main surface of the medium. ;SOLUTION: The hologram photosensitive material 1 is applied on a glass substrate 2.
