A U-shaped pillow (10) provides for the maximization of the cross-sectional area of the air passage of the user at the level of the oropharynx. The user's head is extended backwards in the supine position and the user's occiput rests directly against the underlying mattress. Additionally the pillow has lateral head resting portions (20, 22) forming the legs of the U connected to a central region (26) by a transitional region (28, 30), collectively forming the bottom of the U, that is dimensioned so that the user's thoracic and cervical spine are maintained substantially aligned when the user rolls between the supine and lateral decubitus positions.
