PROBLEM TO BE SOLVED: To provide a membership card application which facilitates the issuance of a membership card and at the same time enables an easy rewriting of the entered items of the membership card. SOLUTION: The membership card application 1 has a membership card retention paper piece 4 which retains a membership card 2 bearing a machine-readable membership code 11 proper to an individual card on the first surface, a name display part 12 of a thermally reversible display part and a privilege information display part 13 as thermally reversible display parts, in a freely removable manner and a duplication paper piece 6 with a duplication entry part 5, these membership card retention paper piece 4 and duplication paper piece 6 being connected to each other with a foldable part 7 as a border. In addition, the application 1 has an identification information display part 16 displaying the same membership code 15 as the membership code 11. Further, a copy paper piece 9 having a membership card application entry part 14 in which member information such as the name of a card applicant, telephone number and the like to be entered by duplication in the duplication entry part 5, is bonded, in a separable manner, to the duplication paper piece 6 along the folding part 7.
