PROBLEM TO BE SOLVED: To make it possible to issue a printed matter, which is hard to forge or alter and to which an individual identification pattern information has been potentially given, and to highly accurately mechanically read the individual identification pattern information on a printed matter at a high speed. SOLUTION: In a forming device A, a photograph of an individual's face 5 is inputted and decomposed into RGB image information so as to produce CMYK printing plate image information for a printing screen at a color image forming part A2. In an ID pattern information forming part A3, an ID pattern information consisting of the feature values of a printing screen form inherent in the individual is formed and registered to a restricted data registering part C. In an ID pattern superposingly forming part A4, a digital color printing image information is formed by superposing the ID pattern information on the CMYK printing plate image information. From an outputting part A5, a printed matter, to which the ID pattern information unrecognizable with a normal viewing is potentially given and which has a photograph of the individual's face, is issued. At a reading device B, an ID information is actualized among issued images in an image processing part B2 so as to compare and collate with the ID information registered in advance in an ID pattern information discriminating part B4 in order to judge whether the printed matter is ture or false, resulting in displaying the result of the judgment in a displaying part B5.
