A clinical examination report has a medical information space where medical information (information on treatment method) is written and an examination result space where data concerning examination result is written. Both spaces are provided on the same recording medium. When the medical information space and the examination result space are provided on the same recording medium, perforations are made between the spaces so that the spaces can be separated by cutting off. When the amount of information to be written in the examination result space is too much, the examination result space can be provided on another paper. In this case, information other than the medical information (information on treatment method) and/or additional information being more detail information is written on the paper along with the medical information space.
