PROBLEM TO BE SOLVED: To provide the method, system and recording medium of a medical chart electronization processing capable of surely securing truthfulness and also securing legibility. SOLUTION: On a pen tablet 8 provided with a display part 81 and a tablet part 82, a chart form as an input picture and the JPEG image of the electronic chart of a previous time as a reference picture are displayed. To the chart form, handwritten characters and sketch images by a stylus pen and text characters/numerical values by a keyboard or the like are written and displayed as medical treatment information. The chart form and the medical treatment information are recorded and preserved in a server 1 or the like by the two kinds of an image file 191 by JPEG and a tagged text file 192. To the two kinds of the files, the same file names for which only expanders are different are imparted. As a file name, the combined one of the identification information of a doctor or a patient and time information for indicating at least to the unit of second as the medical treatment time is automatically constructed.
