MULTIPLE PART GUEST CHECK BUSINESS FORM

A guest check business form will properly alignwith and be read by conventional guest checkprocessing machines. The top part of a first,heaviest ply of the guest check has machine readableindicia (e.g. bar code) printed on it. Below the barcode second and third plies are attached to the firstply adjacent their top edges, with the bottom edgesof all of the plies substantially in alignment. Theadhesive attaching these plies is staggered so thatat no point along the form is there adhesive morethan one layer thick. The adhesive may berepositionable adhesive, or permanent adhesive. Withpermanent adhesive it is desirable to provide fineperforations in the second and third plies just belowthe adhesive attachment of them to the first ply.The first ply is preferably carbonless CF paper, thesecond ply carbonless CFB, and the third plycarbonless CB.
