PROBLEM TO BE SOLVED: To provide a plurality of sheets of card integrating type application for point service and a key holder integrating type application for point service, by which a plurality of sheets of point card commonly usable among members of a family or the like can be issued simultaneously. SOLUTION: In a plurality of sheets of card integrating type application 1, which is made into an integral body with a perforation lines 13 as a boundary between a card holding paper piece with point cards 2 having intrinsic machine read information thereon and an application 11 equipped with the predetermined entry columns, two or more sheets of card are held in the card holding paper piece for family use. Common machine read information is recorded in a plurality of sheets of respective point cards 2 and the application 11. A key holder integrating type application for point service has the similar function under the condition that the shape of a front card is made into a key holder type.
