PURPOSE: A form paper and a storage device thereof are provided to recognize the personal information of a provider, to record any information related with the provider, and to rapidly correspond the provider with the related information. CONSTITUTION: The form paper(100) to be provided in the case of a first meeting consists of a name register part(10) printed with the personal information of a provider and a recording part(20) for recording any information communicated with the provider on a front surface, and an adhesion part on a rear surface to allow the form paper to be conveniently attached to a desired place. By comparing the name register part with the recording part, it is possible to rapidly understand the personal history of the provider.
