PURPOSE: To issue a meal ticket efficiently at a high speed by a method wherein unit cutforms are continuously disposed in a paper feed direction and/or a direction orthogonal to this paper feed direction, and at least a meal ticket amount column and a company name column are provided for every unit cutform. ;CONSTITUTION: Unit cutforms 20 are continuously disposed in a paper feed direction and/or a direction orthogonal to this paper feed direction. At least a meal ticket amount column 21 and a company name column 22 are provided for every unit cutform 20. The unit cutforms are determined as a set unit, wherein the meal ticket amounts of the meal ticket amount columns 21 amount to a predetermined amount. For every set unit, mechanically readable seal marks (s), (t) are added. For example, the content of the mark (s) means a start, the content of the mark (t) means a continuation, and a range from the mark (s) to the mark (t) can be determined as a set unit.
