A 4H-pyrido [4',3':4,5] thieno [3,2-f] [1,2,3] triazolo[4,3-a] [1,4] diazepine derivative of formula (I) is used to treat pathological states or diseases in which at least one of the somatostatin reports are involved such as cancers, fibrosis, and osteoporosis. In formula (I): W is H or R-X-C(Y)-; R is optionally substituted aryl or heteroaryl; X is -(CH2)n-Z; Z is a covalent bond, NH, O or S; n is 0 to 2; Y is O or S; R1 is H, OH, halo or an optionally substituted alkyl or alkoxy; R2a and R2b is H or optionally substituted alkyl, alkenyl, alkynyl or R21Z21; Z21 is O, C(O), OC(O) or S; R21 is H, alkyl, aryl or arylalkyl; R3 is H, nitro, halo, cyano, heteroarylalkyl or optionally substituted alkyl, alkenyl, alkynyl, cycloalkyl, cycloalkylalkyl, aryl, arylalkyl, aryloxyalkyl or heteroaryl or R31Z31; R31 is H, alkyl, aryl or arylalkyl and Z31 O, C(O), OC(O) or S.
