PPROBLEM TO BE SOLVED: To design business forms from which information respectively written on them can be captured without any trouble even though there are a plurality of types of the business forms. PSOLUTION: A business form picture capturing frame setting part 13 sets an area for capturing, as a picture image, a design prepared by a business form design preparing part 11, a business form picture capturing frame information generating part 14 generates information for designating the area as two-dimensional code information, and a business form design outputting part 15 assembles the two-dimensional code information into the prepared design and outputs the design as a business form design.
