The invention concerns a self-retaining retractor (1) for perineal surgery of a patient in gynecological position comprising two globally C-shaped frames (2, 3) of similar or different dimensions mounted mutually upside-down and freely pivotable relative to each other through their ends (201, 202, 301, 302), one first frame being urged to be placed on the patient's abdomen, the second frame being urged to be suspended opposite the perineum, the angle then formed by the two frames (2, 3) being fixed by clamping means (41, 42), said frames (2, 3) being provided on their outer edge with a plurality of notches (6) designed to position and maintain the parietal traction elements (8, 9, 10). The invention is characterized in that at least first means adjacent (11, 12, 13) to the abdominal frame (2) extend on either side of the patient's abdomen lowering the center of gravity of the retractor (1) thus equipped, beneath the operating site. In one embodiment, a valve-holder (8) rod (15) is provided connecting the articulations of the two frames (2, 3).
