PPROBLEM TO BE SOLVED: To provide a traditional Chinese medical physical constitution determination sheet and a traditional Chinese medical physical constitution determination system for simply, exactly determining a traditional Chinese medical physical constitution. PSOLUTION: The traditional Chinese medical physical constitution determination sheet comprises: a physical condition column 2 enumerating a plurality of physical conditions for specifying the traditional Chinese medical physical constitution; matrix-displayed physical constitution columns 3, 4 enumerating a plurality of traditional Chinese physical constitutions, including the presence of physical strength, bad circulation of blood, and blood stagnation; an affirmation entry column 5 for entering affirmation marks when the physical condition and the physical constitution are positively applied to each other; and a denial entry column 6 for entering denial marks when the physical condition and the physical constitution are negatively applied to each other; an affirmation number totalization column 7 for entering the totalization value of the number of affirmation marks entered in the affirmation entry column; and a denial number totalization column 8 for entering the totalization value of the number of denial marks entered in the denial entry column.
