The device has a hollow end (8) with a threaded hole for passage of a screw traversing a hollow elongated central core (6) to be screwed in an implantable medical device (123). Four cutting blades (1) extend longitudinally on an outer surface of the core and have transversal sections in which rays (3, 3`) passing through a long end of the blades and a center (XX`) of the device (123) are greater than a ray (4) of the device (123).
