APPLICATION PAPER FOR INSURANCE CONTRACT, AND METHOD AND APPARATUS FOR FORMING GUIDING INFORMATION FOR INSURANCE CONTRACT

PPROBLEM TO BE SOLVED: To smooth the flow of understanding, judgment and action of a customer to an insurance contract. PSOLUTION: An application paper for the insurance contract comprises a guide book and an application form. The guide book explains first and second contract courses. In the guide book, a first frame line enclosing a range expressing a relative relation of a covering range of the first contract course to a covering range of the second contract course, and discriminating the first contract course, and a second frame line enclosing a range expressing a relative relation of a range of the second contract course to a range of the first contract course, and discriminating the second contract course, are arranged and printed under an embodiment including a frame line enclosing a narrower range inward. In addition, the first and second contract courses are enumerated and printed, and a hole for directly writing a selection of either the first or second contract course into the application form is provided in adjoining to printing. In the application form, at a position corresponding to the hole of the guide book, a column for performing a writing expressing the selection of either the first or second contract course is prepared.
