An apparatus (15) configured and adapted for insertion through a small incision in the body for dissecting, separating or retracting body tissue, comprising a portion (18) of balloon housing having a tubular member (17) connected to a distal end thereof, and a ball (16), wherein one end of the ball (16) is connected sealingly to a distal end of the tubular member (17) and the balloon extends from said distal end back to through said tubular member in said portion (18) of balloon housing, and the apparatus (15) is provided with an inflation port (21) to inject any inflation medium into the apparatus (15) for inflating the balloon (16), so that the ball is forced to reverse out of the tubular member (17), it characterized in that the apparatus further comprises a member (32) of piston disposed in the portion (18) housing ball forming a fluid tight seal with the m isma, wherein the member (32) piston can reposition the portion (18) of balloon housing and an opposite end of the ball (16) is connected in sealing engagement with the member (32) of the piston; including said piston (32) a guide tube (33) to facilitate passage of an endoscope through said piston, the said ball and said tubular member.
