The device comprises means for detecting in the heart rate, the occurrence of a ventricular extra-systole (esv). It further comprises means for measurement of intracardiac impedance (z), which is correlated to the instantaneous blood flow, and means for determining, in response to variations of the impedance signal, the presence or absence of a significant mechanical activity of the myocardium, following the ventricular extra-systole, and to deliver in response an indicator of the character with or without the ventricular extra-systole. This indicator makes it possible, inter alia, to the following cycle, to adapt in a differentiated manner, an operating parameter such as the ventricular escape interval, as a function of the character with or without the ventricular extra-systole.
