PROBLEM TO BE SOLVED: To provide an affiliation system in which an input from a portable telephone is simplified.;SOLUTION: A user fills such user information as his or her address, name and telephone number in an application form 8 installed in a store, and transmits the user information filled in the a host server 11 at a store side and application form ID predescribed in an application form 8 to the host server 11. The user starts a mailer by holding a portable telephone 3 over a reader/writer 9, and transmits mail to which the application form ID of the application form 8 is input to the host server 11.
