A holder (1) has pockets that receive corresponding replaceable cutting inserts (3,5,7). The pockets are arranged to allow indexing of a tool body about an indexing axis to bring one insert into operative cutting position. One insert has a main plane laterally oriented relative to the longitudinal center axis of the holder.
