An anthocyanin extracted from black soybean seed coat is provided to improve survival of flaps or tissues from ischemia-reperfusion injury due to anti-oxidative and anti-inflammatory characteristics, thereby being usefully used as an agent for preventing and treating the ischemia-reperfusion injury. A pharmaceutical composition for preventing or treating ischemia-reperfusion injury in flaps or tissues comprises anthocyanin extracted from black soybean seed coat, wherein the anthocyanin consists of cyanidin-3-glucoside, delphinidin-3-glucoside and petunidin-3-glucoside.
