The instrument (1) has an optical device (2) with a microcamera (4) e.g. complementary MOS image sensor, and a display screen (5) e.g. LCD screen, and a light source (6) for front lighting a work zone. A mechanical interface (7) removably fixes a speculum aligned in an optical axis of the microcamera. A removable/retractable tubular element (8) has a free end with a cutaneous support edge (9) and an observation aperture, and a light source formed of LEDs for lateral lighting the zone. The element allows observation of skin, and forms an opaque chamber in field of microcamera during usage.
