An apparatus (10, 300, 400, 500, 600, 700) to facilitate the performance of a procedure gastroplasty, comprising: an outer guide member (12, 302) dimensioned for insertion and passage through the esophagus a patient, such that the outer guide member having proximal ends (18) and distal (20) and defining an opening (16) therein extending at least along a portion of the length of the member outer guide; an elongated member (14, 404) of input anvil at least partially disposed within the opening of the outer guide member, such that the delivery member is adapted for longitudinal movement within the outer guide member, between a initial position and an actuated or actuated position; and an anvil (32, 306, 604) operatively couplable with the delivery member, so that the anvil includes a stem (46, 402, 410, 538, 616) of anvil defining a longitudinal axis and a head (44, 408, 610) anvil, connected or attached to the anvil rod, so that the anvil rod extending distally from the anvil head in order to engage with a structure corresponding mounting, being the anvil head disposed at least partially within the opening of the outer guide member when in the initial position of the delivery member and being fully exposed or exposed from the distal end of the outer guide member upon movement of the delivery member to the activated position.
