A hybrid integration structure of a planar light wave circuit and an optical active device using an optical fiber array is provided to receive a light irradiated in a light waveguide to a photodiode by using an optical fiber array and an arrangement process. A module structure of a planar light wave circuit comprises an optical fiber(100), an input optical fiber array, a planar light wave circuit(400), a photodiode(500), and an output optical fiber array(300). The input optical fiber array transmits an optical signal transmitted through the optical fiber to the planar light wave circuit. The optical fiber and an input light waveguide of the planar light wave circuit are bonded by forming a V type groove on a substrate(320). The optical signal transmitted from the input optical fiber array passes the planar light wave circuit having a plurality of planar light waveguides. The planar light wave circuit includes a substrate(150), a core(120), and clad layers(130, 140). A resin layer(610) is formed on the planar light wave circuit in order to mount the photodiode. The photodiode is mounted by using a guide groove of the planar light wave circuit.
