METHOD FOR RECONSTRUCTION OF ANTERIOR ABDOMINAL WALL IN POSTOPERATIVE GIANT VENTRAL HERNIA

A method for reconstruction of the anterior abdominal wall in postoperative giant ventral hernia comprises the longitudinal cut of the sheaths of rectus muscles of abdomen along the hernial orifice medially, predominantly in the area of the anterior wall, the lateral mobilization of the rectus muscles of abdomen to spigelian line, the anatomic separation of aponeurotic parts of the internal oblique and transverse abdominal muscles in the area of spigelian line laterally. The posterior leaves of aponeuroses of rectus muscles of abdomen are sutured with the peritoneum. The polypropylene implant is placed below rectus muscles of abdomen medially as well as between internal oblique and transverse abdominal muscles laterally. The anterior leaves of aponeuroses are sutured to polypropylene implant medially generating diastasis between rectus muscles of abdomen depending of the tension of the tissues and the increase in intraabdominal pressure.
