A care plan manager (84) dynamically configures or modifies at least one of the goal module (78, 198, 298) and content sessions (80, 82, 200, 202, 204, 224, 300, 318) based at least on the one input and intervention rules so that the patient's progress toward the projected health management goal is optimized.
