PROBLEM TO BE SOLVED: To provide a two-dimensional collimator module which can be assembled easily with high positional accuracy.;SOLUTION: A two-dimensional collimator module comprises a plurality of first collimator plates 11 arranged in a CH (channel) direction, a plurality of second collimator plates 12 arranged in an SL (slice) direction, and first and second blocks 13, 14 sandwiching the first collimator plates 11 in the SL direction. The plurality of first collimator plates 11 include a plurality of slotted plates 11a, each having slots 111 extending in the direction of radiation formed in plurality along the SL direction, and a non-slotted plate 11b. Each of the slotted plates 11a is placed along a border, in the CH direction, of each radiation detection element of a radiation sensor module corresponding to the two-dimensional collimator module, and the non-slotted plate 11b is placed along an edge, in the CH direction, of the radiation sensor module.
