PPROBLEM TO BE SOLVED: To provide a coupon booklet with an address sheet which eliminates the need to collate an address sheet with coupon booklet when enclosing in a window envelope by filing an address sheet on the top ticket and arranging a front cover and a back cover, and to provide a method for using the coupon booklet with the address sheet. PSOLUTION: In the coupon booklet with the address sheet in which a plurality of coupons with duplicate are filed in piles, at least, the address sheet displaying an address information for sending and an information related to the filed coupon is filed on the top ticket of the coupon booklet.
