A shield nose (40) is affixed to a distal end of the support tube. The shield nose (40) defines a truncated cavity and having a concave center portion. An obturator member (72) is movably mounted relative to the support tube. A cutting blade (74) is provided having at least one cutting edge and is affixed to a distal end (76) of the obturator member (72). 1o The cutting blade (74) is movable within the truncated cavity.
