The invention relates to a method for custom-making a preformed implant, arranged to fix with a screw a first part of a bone to a second part of this bone which must be separated by an osteotomy, and to a preformed guide for drilling holes for the screws and for guiding the osteotomy pattern, said method comprising the steps of, prior to the osteotomy, using a computer to perform a pre-operative model of the osteotomy; complete bone, constituting a pre-operative form of said complete bone, and modifying said pre-operative model, so as to create a modified model. The method is characterized in that it further comprises the steps of creating: 1) on the modified model, in addition to the drill guide holes for the screws provided for the second part of the bone and to trace the osteotomy, additional drill guide holes for the screws for the first part of the bone, 2) on the preformed guide additional holes for drilling guide for the screws for the first part of the bone, and to make so that the holes provided in the implant for the passage of the screws for attachment to the first portion of the bone coincide with the additional drilling guide holes created on the modified model.
